1) Classical combination (Boericke/Kent style) - Intense nighttime itching with burrows, excoriations, secondary infection

Indicated when there is severe itching worse at night and from warmth, with linear burrows in finger webs, wrists, waist, genitals; excoriated papules/vesicles; may have impetiginization from scratching.

Sulphur 200C + Psorinum 200C + Hepar sulfuris 30C

• Dose: 2-3 drops or pellets of each together under the tongue once daily for 2–4 weeks; in acute severe itching, give Sulphur alone every 2–3 hours while awake for 24–48 hours, then return to once daily.

• Keynotes:

  • Sulphur: hallmark scabies remedy—intense itching worse heat/night/warm bed; red, inflamed eruptions; worse washing; often first prescription in classical cases.

  • Psorinum: deep‑seated, intractable itching with dirty‑looking skin; worse cold air; useful when scabies recurs or fails to respond to other remedies.

  • Hepar sulfuris: hypersensitive, painful, suppurating lesions from scratching; chilliness; indicated when secondary bacterial infection (impetigo) develops.

• Contents: 3 units of 30ml drops or 1 unit of 2 dram medicated pills

2) Banerji protocol combination - Scabies 

Indicated for persistent or recurrent scabies where you want a simple, protocol‑style approach alongside standard scabicide treatment.

Psorinum 200C + Coffea cruda 200C

• Dose:

  • Psorinum 200C: 2-3 drops or pellets once weekly for 3 months (primary Banerji remedy for scabies; may increase to twice weekly if no improvement after 2 weeks).

  • Coffea cruda 200C: 2-3 drops or pellets every 2 hours as needed for intense itching (especially nighttime itching that prevents sleep).

  • For severe itching unrelieved by Coffea, add Sulphur 30C: 2-3 drops or pellets every 2 hours as needed.

  • Opening dose (if following strict Banerji protocol): Before starting, give Camphora 200C as a single dose (2-3 drops or pellets), wait 24 hours, then begin Psorinum/Coffea.

• Contents: 4 units of 30ml drops or 4 unit of 2 dram medicated pills

• Rationale: this Banerji‑style pair targets the scabetic miasm and neural hypersensitivity, combining Psorinum’s deep action on chronic parasitic skin disease with Coffea’s specific relief of intolerable itching and sleeplessness

General rules for both: keep doses 30 minutes away from food/drink/brushing; apply prescribed permethrin 5% cream from neck down (repeat in 7 days) or take ivermectin as directed; treat all household members simultaneously; wash bedding/clothing in hot water and dry on high heat; items that cannot be washed should be sealed in plastic bags for ≥72 hours; avoid close skin‑to‑skin contact until treatment is complete; if crusted (Norwegian) scabies, immunocompromise, or treatment failure occurs, seek urgent dermatology care (may need multiple ivermectin doses plus topical agents).

Homeopathic treatment of scabies stimulate the disease-fighting mechanism of the body to fight scabies says Dr Vikas Sharma. They cure the condition safely without any side effects. Dr KS Gopi recommends the following remedies for scabies

• Sulphur 200 is the top remedy for the treatment of scabies (antipsoric remedy). Sulphur is prescribed when intense itching and scratching is present which worsen with washing and heat, especially during the night (because at night, your skin is warmer). It is characterized by  silvery lines (burrow markings) under your skin. Sulphur patients have an unhealthy skin with pimply eruptions which are painfully sensitive to air, wind and washing. If itching after hot shower 
• Causticum 200 is another effective medicine for scabies , especially in the folds of skin and between the thighs. There is severe itching and scratching in the finger webs leading to soreness. Scabies like warm places, such as skin folds, between the fingers, under fingernails, or around the buttock or breast creases. Burning, rawness and soreness are the main symptoms of Causticum in scabies cases.
• Sepia 200 is an effective remedy for scabies and prescribed when the patient complains of itching vesicles (a small fluid-filled bladder, sac, cyst, or vacuole within the body) not relieved by scratching. Scabies  rash also can include tiny blisters (vesicles) and scales. The blister is caused by the mite burrows getting infected by bacteria Crusted scabies is characterized by vesicles and thick crusts over the skin that can contain many mites. Other symptoms: The skin turns pinkish when scratching. The condition gets worse in the open air while the person feels better in warm room. Scabies is mainly present at the bends of the elbows and knees.
• Arsenicum album 200 is best for scabies and prescribed when intense itching along with great restlessness is present. The unease and agitation is due to sleeplessness due to itching that tends to get worse at night. Other symptoms: The skin peels off in large scales and is very sore to touch. The skin is dry, rough, scaly, dirty and burn intensely .The person is unable to sleep due to violent itching in the affected areas. Over sensitiveness of the skin with acute itching and burning also observed.
• Graphites 30 is best for scabies and it is prescribed when the skin is rough, hard, dry and unhealthy. The dry and roughness of skin is caused by excessive scaling and flaking of the skin. Every little injury suppurates. From the eruptions ooze out a sticky exudation. There is rawness in bends of limbs, groins, neck and behind ears.
• Psorinum 200 is one of the best remedies for scabies where the skin is unhealthy and there is an abnormal tendency to skin diseases. The intense itching of scabies leads to scratching can cause skin sores. There is severe itching which gets worse from lying in bed. The itching is so severe , so the person scratches until the skin starts to bleed.The skin is dirty, rough, scabby and greasy.
